It would be helpful before your visit if you would take time to open the links below, print out the welcome to the office and medical history form and fill them out. If you bring them with you on your first visit it will save lots of time and enable us to get you established and seen on time.

Also, kindly bring any insurance cards or information with you on the first visit as well.
